Animal Incident Report

An Animal Incident Report (AIR) must be filed with the Animal Care Committee for every incident involving an animal listed on an AUP that is outside of the expected conditions of that AUP. If, at any time, there is doubt as to whether or not to submit an AIR, the default action is to submit.

This form is used to request access to the facility(s) and any keys that may be required. Before Access can be granted, all users must be added to an Animal Utilization Protocol (AUP) and have completed a Central Animal Facility and/or Isolation Facility Orientation.
