Course instructors are to complete and submit this form to initiate review of pedagogical merit for Animal-Based teaching or training. For certain competency-based teaching or training activities where the intended learning outcomes are prescribed or mandated by a third party, such as a ministry of education, an institutional research/personnel training program or an accreditation or certification body, an expedited review process is available.

The Central Animal Facility is a controlled access facility requring iClass card entry.

This form is used to request access to the facility for the purpose of volunteering. A orientation to the facility and completion of the Animal Care Core Modules is requried prior to card activation.
